Hairloss and Irondeficiancy - Haarausfall und Eisenmangel

Für alle, die sich eigenes, langes Haar wünschen oder dieses schon haben, gibt es neben Frisören, die die Schere zu großzügig ansetzen, ein großes Schreckgespenst: Haarausfall. Ihr findet hier bei mir auf der Hairloss Help & FAQ-Seite viele allgemeine Infos, in der Kategorie "Hairloss" außerdem viel von meinen persönlichen Erfahrungen.
Heute möchte ich allgemein auf die mit Abstand häufigste Ursache eingehen, wenn es um durch Mangelzustände verursachten Haarausfall geht: Eisenmangel.

Today I want to speak about about one of the most common causes for hairloss due to deficiancies: I want to talk about hairloss due to irondeficiancy.

Was ist Eisenmangel?
Haarausfallverursachender Eisenmangel ist in der Regel im Blutbild als niedriger Ferritinwert zu erkennen. Ferritin? Genau.
Eisen ist ein Mineralstoff, der im Körper für verschiedene Funktionen benötigt wird. Die prominenteste ist der Sauerstofftransport im Blut, die für uns hier interessanteste ist seine Beteiligung am Haarwuchs.
Verschiedene Eisenwerte lassen sich im Blutbild darstellen: unter anderem Eisen im Serum, Hämoglobin und besagter Ferritinwert, angegeben in µg/l oder ng/dl (kommt aufs selbe raus).
Je nach Quelle sollte der Ferritin zwischen 12-70 Minimum und 200-400ng/dl Maximum liegen.
Ich halte mich an meinen Hautarzt: "Wer einen Ferritin unter 40 hat, der muss sich nicht wundern, wenn die Haare ausgehen."
Der Wertebereich zwischen 50 und 70ng/dl ist eine gewisse Grauzone, in dem viele völlig gesund sind, andere bereits unter Mangelerscheinungen leiden. Hier gilt: Wer Symptome hat, sollte aufsättigen, wem es gut geht, sollte einfach ein Auge darauf behalten (entsprechende Ernährung, Kontrollblutuntersuchung), dass es nicht weiter absackt.
Ziemlich einig sind sich alle Quellen, dass ein Wert von um die 100ng/dl recht optimal ist.
What is irondeficiancy?
Irondeficiancy which results in hairloss can usually be diagnosed by having a look at your blood levels and here the most important actor is low ferritin. Ferritin? Let's fix that: iron is a mineral which is used in the body for various jobs. The most important job is to transport oxygen with your blood. The most interesting job for us is of course iron's role in hairgrowth. There are different levels which can be measured in your blood to check your iron (and now translation becomes hard...): Iron in  the serum, haemoglobin, some others and the before mentioned ferritin which should range between 12-70 minimum and 200-400ng/dl maximum, depending on the source you ask. I go with my dermatologist who told me: "With ferritin lower than around 40 there is no wonder you are shedding like crazy."  The range between 50 and 70 seems to be highly individual if it causes already hair problems or not. Some people live very well with full hair around 50ng/dl, others already shed. Here you should decide by the symptoms: if you are shedding, than supplement. If not, fine. Than don't force artificial nutrients into your body but keep having a look at your levels every now and than and maybe check your diet to improve iron intake. Even though the sources are unshure about what is considerd to be the lowest stil acceptable level, they all agree that a level around 100 ng/dl is fairly optimal for your health.

Wieviel Eisen brauche ich und wo kommt es her?
Eisen wird dem Körper als Eisenkomplex zugeführt. Nägel essen bringt also nix.
Es gibt Eisen-II-gluconat (meist tierischen Ursprungs) und Eisen-III-gluconat (meist pflanzlichen Ursprungs). Letzteres wird schlechter verstoffwechselt, weil dafür erst ein Enzym aktiv werden muss, welches das Eisen-III zu Eisen-II reduziert.
Zum Vergleich:
3-8% des pflanzlichen Eisens werden aufgenommen
~20% bei tierischem Eisen.
Frauen haben laut der Deutschen Gesellschaft für Ernährung einen Eisenbedarf von 15mg/Tag, der über die Nahrung aufgenommen werden sollte.
Männer 10-12mg. Der Unterschied kommt unter anderem durch die Regelblutung (und wohl auch durch hormonelle Unterschiede bei Männern und Frauen allgemein, Östrogen und Testosteron...). Frauen brauchen also 20-30% mehr Eisen als Männer! Die Menstruation ist nicht nur natürlich sondern leider auch einfach ein montalicher Blutverlust, der ausgeglichen werden muss. Schwanger oder stillend ist der Bedaf noch mal höher.
How much iron do I need and how do I get it?
Iron is resorbed by your body as an ironcomplex. So no worth eating nails. There are iron-22-gluconat (mostly of animal sources) and iron-III-gluconat (mostly of plant sources). The lkkast one is more difficult to break down for our body, as it needs enzymes to reduce it fromn iron-III to iron-II.
For comparison: 3-8% of plantbased iron is absorbed but around 20% of animalk based iron. 
Due to the German Society for Nutrition women have a daily need of 15mg while men need about 10 to 12mg. The difference comes from the monthly mestruation. You see, women need 20-30% more! Menstruation is not only natural but simply a monthly loss of bood which is needed to be balanced! Pregnant or nursing the need of iron is even higher.

Symptome
Diffuser Haarausfall, der oft schleichend beginnt und rasch sehr heftig werden kann (mehrfache dreistelliger Bereich an Haarverlust pro Tag, bis in die Tausender hinein). Der Arzt spricht von "telogenem Effluvium", also hohem Haarverlust durch einen verkürzten Wachstumszyklus. Der Eisenmangel hat dazu geführt, dass Haar, das eigentlich noch hätte älter werden können aus der weiterwachsenden Anagenphase in die abgestorbene Telogenphase geschickt wurden (mehr zu den Haarwachstumsphasen findet ihr hier). Die Telogenphase ist nicht umkehrbar. Haare die abgestorben sind, werden ausfallen.
Häufig in Kombination mit Müdigkeit, Konzentrationsschwäche, brüchigen oder gummiartigen Fingernägeln, gräulichen Augenringen, Mundecken und Infektanfälligkeit. Also leider Symptome wie von zig anderen Krankheiten auch.
Symptoms: Hairloss all over the head, not restricted to one area. The shedding starts blending in but can increase very fast to hunders of hairs a day, maybe even over 1000 hairs a day. Your doctor is talking about "telogen effluvium" now, as you are shedding hair which is in the telgone phase (more about the hairgrowth phases can be found here). This means, the irondeficiancy stopped the hairgrowth too early, the hair is already dead for weeks, when you finally shed it. The telogen phase is irreversible. You cannot save your already telogen hair. So your aim is to stop that process of too early hair death and protect the anagen hair you still have. The telogen hair will go, no matter what you do. Besides shedding, irondeficiancy is often accompanied by exhaustion, lack of concentration, brittle nails, dark underyey circles, sore liable angles and being prone to flue bugs. That's difficult, as theses symptoms come alongside so so many illnesses.

 
Mögliche Ursachen für Eisenmangel
Die Ursachen sollten unbedingt zusammen mit einem Arzt abgeklärt werden, auch um schlimmeren Folgen als "nur" Haarausfall zuvor zu kommen.

Theses causes should definitely be checked with your doctor because symptoms can get much worse than "just" hairloss.


Medizinische Behandlung
Natürlich sollte die Ursache gefunden werden, also das Leck, worüber Eisen verloren geht oder der Grund, weshalb nicht genug im Körper kommt. In der Regel werden zunächst aber gegen die Symptome Tabletten/Retardkapseln verschrieben. Verträgt man die, sind sie Mittel der Wahl. Wenn nicht, oder wenn es noch schneller gehen muss, sind Eisenspritzen und -infusionen eine Möglichkeit.
Eine eisenreiche Ernährung ist zum raschen Auffüllen der Speicher nicht geeigent, da es Monate bis Jahre dauern kann über Ernährung die Speicher wieder aufzufüllen, ist aber sehr gut, um auf lange Sicht keine Supplemente mehr zu benötigen und natürlich als Prävention.
Medical treatments: Of course it is crucial to find out what is causing the deficiancy: where you are loosing iron or why you just don't get enough in. But in general you can get supplements as first treatments against your symptoms, usually in form of pills or extended-release capsules. If you can work with them, fine. If not or if is important to supplement quicklier you can also get injections or infusions. A diet rich in iron is not what you need to fill your reservoires quickly as this might take months or even years. Of course diet is essential in the long run to wean off the supplements again, but for a quick fix it is just too slow.


Nahrungsergänzungsmittel
sind eher etwas für die Psyche in der akuten Leidenssituation oder präventiv. Man tut ja was. Eher für das gute Gefühl und den Geldbeutel des Apothekers und Drogeristen. Legitim in meinen Augen dennoch, da Haarausfall vor allem die Psyche belastet. Körperlich ist er ja schmerzfrei. Aufzuzählen wären:
Bierhefe, Floradix Kräuterblut, Rotbäckchensaft, Amecke Eisen plus, Nutrilite Eisenkautabletten und andere Präparate. Ärzte grinsen über sowas in der Regel nur milde, da die Dosierung sehr gering ist, zumal für recht würzige Preise. Nehmt da dann aber nicht einfach nach Gefühl dann mehr. Ihr wollt euch etwas Gutes tun und bei solchen Konzentraten gehört das in Ärztehand und kontrolliert. Mein Rat: Versucht lieber richtig viel eisenreiche Kräuter zu essen (siehe unten). Damit macht ihr nichts kaputt, im Gegenteil. Bedenkt, dass ihr euch wirklich hauptsächlich einen Placebo und viel Trägersubstanz kauft. Auch zu viele Supplemente können schaden. Also: lieber beim Arzt und fürs Gefühl richtig gutes Essen und tolle Haaröle.
So let's talk about supplements you can buy at the drugstore. Let's state it: these pills are more for emotional balacnce than for your blood levels. Their dose is too low compared to real medicational supplements. Doctors usually only smile about these products. But if you need that feeling of "doing something", maybe you can go for it. But don't level up the dose only because I said the pills are too low in iron. Artificial supplementation is something which should be checked by your doctor and too high supplementation can cause damages too! My advice: better try to eat as much herbs rich in iron as possible and take good care of your scalp for that feeling of really fighting the enemy. That won't do you any wrong. See below.

Ernährung
Präventiv und in der akuten Situation unterstützend sehr nützlich, akut aber nicht ausreichend, um schnell gegen die Symptome vorgehen zu können.
Diet: Preventively or supportingly this is really the way of choice. But in the acute deficiancy situation diet alone is not sufficiant to help you overcome your troubles quickly.

Eisenreiche Nahrungsmittel
(willkürliche Reihenfolge):
Kräuter, die mit Abstand das meiste Eisen liefern können, werden in der Regel nur als Tees oder Gewürze verwendet und somit ist es damit schwer auf die nötigen Mengen zu kommen. Grüne Smoothies und echte Kräutersalate sind hier eine gute Möglichkeit mehr nährstoffreiches Kräutergrün in die Mahlzeiten zu integrieren. Also mal nicht nur ein Dekopetersilchen an den Tellerrand legen, sondern gleich den ganzen Strauß essen.

Foods rich in iron (random order): 
Herbs are by far the most powerfull ironfoods, but in conventional diets they take only place as teas or like spices. I recommand eating real herb salads or green smoothies. Just eat the whole bunch of parsley not only the tiny leaf as decoration.

Vitamin C and andere organische Säuren: Weinsäure, Zitronensäure, Essigsäure, Milchsäure
Auch hier: Smoothies und Salate sind optimal da sie das Vitamin C gleich mit liefern.
Supportive to resorb iron (so eat in combination with the foods above): vitamin C and other oragnic acids such as: tartaric acid, citric acid, acetic acid, hydroxypropionic acid. Same here: smoothies and salads ar optimal, as they give you both: foods dense in iron and vitamin C.  

Hemmt Eisenaufnahme
(1-3 Stunden Abstand zu den Eisenlieferanten wären ideal):
Generell konkurrieren die metallischen Spurenelemente im Körper um ähnliche Stoffwechselprozesse bei der Aufnahme. Deswegen ist es so wichtig, dass ihr immer abwechslugnsreich esst und auch nicht dauerhaft supplementiert. Dauerhafte Eisensupplementierung führt sonst leicht in den nächsten Mangel wie z.B. einen Kupfermangel. Auch deshalb: wenn Supplemente, dann haltet euch an die zeitlichen Abstände zu anderem Essen. Nur so seid ihr sicher sowohl die Supplemente als auch euer Essen optimal ausnutzen zu können.


Inhibit iron resorbtion (eat only 1 to 3 hours apart from your foods dense in iron or your supplements):
In general the metallic micronutrients compete for the same or similar resorbtion processes. That is why it is so important to eat an abundance and not to eat everyday the same foods, even though they look healthy at first glace. And that is why it is so important not to supplement over a long term as a long iron supplementation can for example easily lead to copperdeficiancy.  You see why it is so important to seperate your intake of supplements from your normal daily meals.

Haarpflege bei Eisenmangelhaarausfall
Dieser Haarausfall hat eine innere Ursache und somit ist alles von außen nur ein bisschen was fürs Gefühl. Wie bei allen Haarausfallarten gilt: keine zusätzlichen Reize wie Hitze, Chemie oder mechanische Beanspruchung. Sanftes Reinigen, schonende und nährende Kopfhautpflege. Ihr könnt das Eisen nicht über die Kopfhaut aufnehmen, das muss von innen geschehen, aber ihr könnt eure Kopfhaut so behandeln, dass sie möglichst entspannt und gut versorgt ist, das schadet nie und hilft den Kopf vorzubereiten auf den kleinen Neustart, den er hinlegen soll, wenn der Mangel behoben ist und die Haare nachwachsen.
Haircare with irondeficiancy hairloss: This hairloss has an inner cause, so treatments from the outside are just to help you feel better. It counts the same as for other hairlosses: no extra stress from heat, chemicals and rough brushing, teasing or alike. Be gentle, wash with mild shampoo or soap, give your scalp nuriting care with pure oils. These oils will help prepare the scalp for the newstart which should be necassary after the shed, when your hair starts regrowing. And a healthy scalp is never wrong when you want long, healthy hair.

Was kann ich sonst noch tun?
Ich werfe mal wieder die Gebetsmühle an: lebt gesund. Ernährung ist bei solchen Nährstoffmängeln der wichtigste Punkt, den ihr angehen solltet, aber nicht der einzige. Leben ist mehr. Bewegt euch an der frischen Luft, im Tageslicht. Vermeidet Stress, bzw. lernt einen Umganag mit Stress, der euch nicht stresst. ;)

Wie ich euch hier schon erzählt habe, war mein finaler Absprung von Eisensupplementen erst möglich, seit ich mich nicht mehr nur aufs Essen fokussiert habe, sondern meinen Schweinehund überwunden habe und angefangen habe Sport zu treiben. Und zwar regelmäßig. Und nicht nur so ein bisschen. Stress ist für mich auch ein ganz wichtiger Punkt, an dem ich noch arbeite. Kümmert euch um euch. Euer Körper wird es euch danken. Auch wenn eure Haare vielleicht viel Geduld von euch verlangen werden. Ich musste Jahre warten. Jahre. Deswegen sage ich: akut bitte zum Arzt. Das Langzeitproblem, das geht ihr dann mit Muße an.
What can I do in supportingly? Oh well you have heard that so often from me: live a healthy life. Of course a healthy diet is the base when you deal with hairloss due to deficiancies, but diet is not the answer to everything. Move your body outside in daylight and fresh air, avoid stress or at least find a way of dealing with stress, so that it does not stress you that much. ;) Don't ask for a food answer if you have a lifestyle question. Quote from Dallas and Melissa Hartwigg, "It starts with food".
As I already told you, for me it made the last significant difference to start focussoing on more than "just" diet, to wean of my iron supplements, when I started working out already over a year ago. And I mean real workouts. Regularly and sweaty. Stress is still a hard topic for me and an important point I am working on. Take good care of your body and your entity will be greatful. And keep in mind that hair needs a lot of time to recover. Months or even years. That is why I say: go see a doctor, when you have iron trouble. The longterm fix can be made step by step then. But at first you need a quick fix.


Nicht aufgeben! Und mit Menschen, die euch verstehen, statt belächeln, ist alles noch mal besser. Denkt immer daran: ihr seid mehr als eure Haare. Und glücklicherweise können Haare etwas, was andere Körperteile nicht so einfach können: nachwachsen.
Don't give up! And with people how understand your sorrows instead of smiling at you, everything gets easier. And keep in mind: you are much mroe than your hair. But luckily hair does one thing which other bodyparts cannot that easy: it regrows.

Fazit
Zum Arzt gehen, Blutwerte prüfen lassen, Symptombehandlung über Tabletten, Spritzen oder Infusionen besprechen und beginnen, Kontrollen nicht vergessen. Herausfinden, was den Mangel verursacht hat und die Ursache beheben. Esst gesund, lebt gesund, und wie immer: keine Panik. :)
Summary: Go see your doctor and get your blood tested. Cure your symptoms with pills, capsules, injections or infusions which will be given to you by your doctor. Follow his/her instructions! And don't forget to check again after taking the supplements to see, if they worked. Find out what caused the deficiancy and get rid of that cause. Eat healthy, live healthy and as always: don't panic.:)
